AUTHORIZATION/ DECLARATION/ CONSENT*
I warrant that all information and documents that I have submitted to Sterling Bank of Asia Inc. (A Savings Bank) (“SBA”) in relation to this application are true and correct. Any misrepresentation or concealment of material information shall cause the disapproval of this application and shall be construed as an act to defraud SBA, for which I may be civilly and criminally liable. I undertake to promptly notify SBA in writing of any material change with respect to all information and documents that I have submitted.
I authorize SBA to make the necessary investigation to verify my identity and creditworthiness. SBA may inquire and receive any information about me, my creditworthiness, or my financial condition from any appropriate source, including, but not limited to, my current and previous employers, credit bureaus, banks, credit card companies and other financial institutions, government agencies and instrumentalities, or private associations of which I am a member. I release any such source of information from any and all bank secrecy and other confidentiality or data privacy obligations, and waive my rights, under our contracts and applicable laws, including without limitation, Republic Act No. 1405 (Bank Secrecy Law), Republic Act No. 6426 (Foreign Currency Deposit Act), and Republic Act No. 10173 (Data Privacy Act).
