Completely filled out Avance Salary Loan Plus application form
Digitally signed Promissory Note and Disclosure Statement (PNDS) and Addendum
Copy of one (1) month latest payslip
Copy of Company ID
Copy of one (1) valid Government-issued ID (click here for the list of acceptable IDs)
Latest Income Tax Return (BIR FORM 2316)
With an existing Sterling Bank Payroll Account
